What is the relationship between FACE forms and Formulate?
We have developed a wide range of assessment and planning tools over the last 20 years, known as FACE forms, including toolsets for personalisation/strengths-based working in social care, Learning Disabilities, CAMHS services and Risk Profiling.
The original work we did to develop Formulate in Adult Social Care back in 2009/10 used the existing FACE Overview Assessment and identified the key ‘scored’ items relevant for resource allocation. This dataset later became a form in its own (Needs Profile).
Over time, Needs Profiles were developed for the other cohorts of people that Formulate now serves, including Continuing Healthcare and Children and Young People’s Social Care, Continuing Care and SEND.
