V9.2 of the Strengths-based Toolset for Adult Social Care is here!
We are pleased to announce the V9.2 release of the Imosphere Strengths-based Toolset.
This release introduces two new forms into the toolset, as well as a number of technical improvements to the other forms. With the new forms, we are focusing on prevention and the front-door - ensuring that the Imosphere Strengths-based Toolset helps you in managing demand and reducing long-term care needs, whilst continuing to take a person-centred approach focused on outcomes and well-being.
Highlights
Building Independence and Strengths Assessment (BISA)
The BISA is a new form within the V9 toolset, designed alongside Sunderland City Council.
This form provides an alternative (or additional) approach to the Preventative Strengths and Needs Assessment - focusing on the identification of preventative interventions, and evidencing the outcomes of those interventions. It is a therapies-first form, designed specifically with the needs of therapies services in mind.
Self-assessment
For the first time the Self-assessment is now available for customers using the Mosaic case management system, and is available as a MOFORM file.
Further changes have been made to the Self-assessment for customers using the LAS case management system, including the introduction of a new setting to enable or disable Formulate and the Needs Profile, as well as a number of technical fixes.
Carer’s Self-assessment
The Carer’s Self-assessment is a new form within the V9 toolset and has been designed to sit within your case management system portal.
The form will give carers the opportunity to access tailored information and advice, check the eligibility criteria, and to tell their story, in their own words, in a time and place that suits them.
The structure of the form mirrors that of the citizen’s Self-assessment, with Part One of the form designed to provide tailored information and advice, and Part Two mirroring the Carer’s SANA and Needs Profile (if Formulate and the Needs Profile questions are enabled).
