What FACE Toolsets are available?

Our Strengths-based Toolset for Adult Social Care includes tools for use with adults with care needs and carers - covering first contact (‘front door’), Care Act assessments (strengths and needs), care and support planning and review, as well as specialist assessments for sensory needs and occupational therapy/reablement.

Beyond this, we currently offer:

  • A suite of best practice clinical risk assessment and planning tools which span older people, mental health, perinatal, learning disabilities and substance misuse groups.

  • A child and young person’s risk assessment suite (CARAS) which includes a risk assessment and planning tool for young people, accompanied by a range of specialist schedules which feed into this.
